While it's true that nearly everything you do in The Expanse: A Telltale Series has some minor effect on the story, there are a handful of decisions that cause enormous changes. As Khan points out in Chapter Two, it can sometimes feel like whichever choice you make will be wrong.

If you're grappling with the implications of a major choice, keep reading to find out what will happen down the road. Of course, we recommend playing through the game once on your own before using this guide to craft your perfect run.
This guide contains major spoilers.
Should You Cut Off Rayen's Leg?
On the Urshanabi, Rayen accidentally detonates an explosive and gets his leg caught under a cargo container. Releasing the container into space means giving up on the supplies inside, but if the puncture in Rayen's suit isn't patched immediately he could die.
If You Cut Off Rayen's Leg:
The crew will accept that you did the right thing, as critical supplies are hard to come by in the Belt. However, Rayen will never fully trust Drummer and you won't be able to get his best ending.
During the final battle in Chapter Five, Drummer will be able to use the weapons that were in the cargo container to fight Toussaint's pirates.
If You Release The Container:
Rayen will thank Drummer for putting his well-being first, but Arlen will call his brother a coward. If you continue to be supportive of Rayen, he will be more likely to help Drummer when she comes into conflict with Arlen later.
Without the container and the weapons inside, Drummer will have to rely on her wits to evade the pirates in Chapter Five. Curiously, this actually makes the fight easier since there are fewer quick-time inputs.
Should You Space Cox?
When Cox betrays Drummer at the end of Chapter One, you have the choice of killing him by launching him into space, or showing mercy and throwing him in the brig.
If You Kill Cox:
Cox will have no further part in the story. He's just dead.
If You Let Cox Live:
Drummer will be able to ask Cox for information throughout the remainder of the game. He won't be especially forthcoming, but it's better than nothing.
In Chapter Two, you can collect a bottle of scotch from Cox's quarters and give it to him in the brig. If you do so, he will help you fight Arlen during the final confrontation in Chapter Five.
Should You Sleep With Maya?
At the end of Chapter Two, Maya invites Drummer to her quarters.
If You Spend The Night With Maya:
Drummer and Maya become a couple for the remainder of the game, changing multiple dialogue lines. Talking to Maya on board the Cassiopeia will trigger new conversations - importantly, the first one will boost Rayen's trust someone as he pokes fun at the couple.
If You Spend The Night Alone:
Maya is hurt by the rejection, but remains loyal to Drummer. She will still seek to prove her worth as an honorary Belter.

How To Save Maya / Should You Trust Rayen?
When Arlen betrays Drummer at the end of Chapter Three, Rayen asks her to trust him as he takes her to the airlock.
If You Trust Rayen:
At the airlock, Rayen will apologize for not being able to stand up to Arlen. If Drummer saved Rayen's leg, broke up the fight between the brothers, and gave Arlen the medicine found in Chapter Two, then Rayen will change his mind and fight back if encouraged to do so. In this scenario, he prevents Arlen from spacing Maya. If Rayen doesn't fully trust Drummer at this point, Maya dies instead.
If You Fight Rayen:
A quick-time event will ensue. Hitting every input perfectly will give Maya a chance to escape. This is the only way to save Maya if you cut of Rayen's leg, or otherwise don't have his full trust.
Should You Kill Rayen?
At the beginning of Chapter Five, Tor holds Rayen at gunpoint and tells Drummer to kill him as a show of loyalty.
If You Kill Rayen:
Tor will continue to escort Drummer through the Artemis, and you'll have to fight him when you reach the bridge. Rayen has no further part in the story.
If You Fight Back Against Tor:
Tor escapes and calls for backup, and Rayen survives with a minor spinal injury. Drummer takes Rayen to Med Bay, where Virgil may or may not patch him up (see below).
On the bridge, Drummer will arrive to find that Khan has already killed Tor, and the two will fight the pirate reinforcements together. If Khan's surgery was aided by the Laser Crystal and you gave her the cigar in Chapter Two, she'll survive the fight provided you hit every input correctly.
Should You Help Virgil?
When Drummer arrives at Med Bay, Virgil is locked in his quarters. He'll only come out if he trusts Drummer; you'll need to have given him the Mushroom Powder in Chapter Two, and asked about his UNN service but ensured him that you're both on the same side.
Virgil reveals that he was present at the Anderson Station Massacre; Drummer can either help him through his panic attack or leave him to his fate.
If You Stay To Help Virgil:
You'll go through a lengthy dialogue where you help Virgil through his panic attack. If you're successful, he'll heal Rayen and Khan's injuries to help them escape, provided they are both alive.If Rayen is alive and is healed by Virgil, you can talk to him before going to confront Arlen. If he fully trusts Drummer, he'll arrive during the fight to try to stop Arlen, triggering his best possible ending.
If You Leave Virgil Behind:
Virgil is unable to help Rayen or Khan.
