When you normally encounter a wrecked ship in The Witcher 3, you'll find a body and a note leading you to some treasure. If you've stumbled on a wreck off Ard Skellig's eastern coast, you'll find a similar proposition. One of the wreck's victims died clutching a letter that will lead you on a cross-continental search.

This starts the quest, From a Land Far, Far Away, which will entangle Geralt in a complicated situation between two people from a distant nation with a child caught in the middle. If you need help with finding these people, this guide will help you.
Where To Start From A Land Far, Far Away
You'll have to reach an island off the coast of Redgill on Ard Skellig to start this quest. On the island, climb up the hill to find a body laying by the tree. It has a journal that indicates that the ship had been tailed by another ship before it was dashed against the rocks in a storm.
To find out more, head to the island's south coast and dive to explore the shipwreck. You'll find a chest containing a sealed letter and the captain's log. The letter itself is indecipherable, but the log gives Geralt a clue.
The crew had sailed alongside another ship, which was due to pick up supplies from Ursten, a settlement south of Novigrad.
Visiting Ursten
Go to the coast southwest of Novigrad to find Ursten. If you want to bring the quest to a swift conclusion, you can travel north of the village, following the coast, to find the survivor.
You can find a warrior standing outside a house on the southern border of the search area. He'll explain, in broken words, that he's looking for a child before the conversation ends. He won't help you find the survivor, though interacting with him will change the ending.
Once you're ready to move on, follow the coast to Widows' Grotto, where you'll find the survivor just inside the cave.
Dealing With The Survivor
You'll find her trying to protect a child when you arrive, either with the warrior already confronting her or arriving shortly after based on your interactions with him. The warrior tells Geralt that he needs to bring the child home forcing you to pick a side.
Side With The Warrior
He will kill the survivor, ending the quest.
Side With The Survivor
You will have to kill the warrior. The survivor will be grateful, giving Geralt a stone medallion before she leaves.
The stone medallion isn't much of a reward, being worth a single crown.
