That doesn't mean that the story's over. Something Ends, Something Begins is the game's epilogue, giving you an ending based on your choices.

There are three variations, based on how Geralt interacted with Ciri. This guide won't explain how to earn each variation, though it will guide you through each sequence. They aren't very hard but if you can't see past the tears, we'll help you wrap the quest up.

This variant begins with Geralt visiting the Royal Palace in Vizima for a tense and uncomfortable meeting with Emhyr. Geralt has the unenviable task of telling the emperor that his daughter, Ciri, had died, and her body will never be recovered. He takes the news as well as one would expect, though he lets Geralt leave with his life.
Your dialogue choices won't matter here. There's nothing Geralt can say that'll make the situation worse.
Geralt leaves, reaching White Orchard, where this story began. He's due to meet a man at the Nilfgaardian Garrison, though the ruins have been abandoned by their inhabitants.
You'll encounter a group struggling with their stuck wagon. They'll give Geralt and update on the war, determined by your choices in Reason of State.
At the ruins, a cutscene begins where Geralt waits for a blacksmith to arrive. The man eventually arrives, giving Geralt a masterfully crafted silver sword. You'll get to choose the blade's inscription before the transaction is complete.
From here, you'll have to travel to the White Orchard Inn, which you can reach by Fast Traveling. Geralt will sit next to a hooded woman, who is none other than Ciri. He presents her with her new sword before the quest ends, with Ciri becoming a witcher.

In this version, the quest begins with White Orchard blanketed in snow during the winter. Geralt, joined by Zoltan, Dandelion, and the sorceress you romanced, is passing time at the inn. At the sorceress' goading, Geralt heads out in the snow.
You can talk to Zoltan and Dandelion before heading out. They'll have some news to share about their personal lives.
The quest will take you out to Mislav's hut, who you met during The Beast of White Orchard. He'll tell Geralt that the person he's looking for has already gone. You'll have to follow the quest marker to the next area.
There, you'll find Ciri sitting on a ridge and watching the rabbit traps. After a short conversation, she asks Geralt to take her to a griffin nest. You'll have to guide her there, following a quest marker.
Geralt and Ciri will talk during their journey, which you may miss if you move too quickly.
At the nest, Ciri lets herself get distracted and nearly falls victim to a forktail. You'll have to kill it before the conversation continues, with signs that something is bothering Ciri.
Wanting to change the subject, Geralt brings Ciri fishing. You'll have to follow the quest marker to a frozen lake where the pair finds a bear trying to catch a fish through a hole. You'll have to throw a bomb near its head to scare it away.
You can save some time by throwing the bomb at the hole immediately.
Geralt will demonstrate the witchers' fishing technique by tossing a bomb into the hole. The explosion will kill several fish, showing the surrounding area with their carcasses which you'll have to collect.
With dinner earned, the pair return to White Orchard where the source of Ciri's distress becomes apparent. She had agreed to become the Empress of Nilfgaard, with today being the last day before she began preparing for the role.
After a heartfelt goodbye, the quest ends.
How To Complete The Worst Ending
If you haven't succeeded in keeping Ciri's spirits up, you'll be confronted with this ending in Velen. Geralt enters Crookback Bog, confronting a werewolf. The lycanthrope will ask Geralt why he's come and caution against it. You'll have two choices here.
"No."
Geralt will draw his sword and battle the monster to the death.
"There's One,"
Geralt will spare the werewolf on the condition that they fight together. The werewolf will agree, accompanying Geralt through the rest of the quest.
Heading towards the Crones, Geralt will find a group of hunters that had captured a wolf. Regardless of the werewolf's presence, you'll have to confront the hunters with all options leading to bloodshed. You'll have to kill all of them before the quest continues.
Reaching the old orphanage, Geralt comes face-to-face with the Weavess, the only one to have survived Ciri's attack. After exchanging words, you'll have to fight the Crone. She'll be assisted by drowners and a grave hag.
She hasn't changed, fighting just as she did at Bald Mountain. She shouldn't be much of a challenge.
After killing her, a final cutscene begins where Geralt searches for Ciri's medallion, ending with him mourning as monsters close in.
