Searching Kaer Morhen's crumbling walls in The Witcher 3 will often yield remnants left behind by its former residents. The side quest Berengar's Blade is one that lets you find a final memento left behind by Berengar, a witcher who perishes in the series' first game.

Not only will the quest let you learn more about a former witcher, but it will also let you earn the diagram for one of the game's deadliest silver swords. Of course, things are never so simple. You will need to find your way to a chort's lair and slay the monster to bring the quest to a close.
Where To Find Berengar's Notes
This quest starts in Kaer Morhen's inner courtyard,west of the grindstone. You will find a half-destroyed room by the wall, with entry granted by a fractured grate. Head inside and search the lone chest to find a note.
Read the note to learn about Berengar's failed attempt to forge a sword and his frustration with Vesemir's advice. With his interest piqued, Geralt will decide to talk to Vesemir to learn more about this weapon.
You must talk to Vesemir before going to The Isle of Mists. He will not be available after its conclusion, preventing you from completing this step.
Vesemir gladly tells Geralt about Berengar's misadventure, noting that the sword was never completed. Not all hope is lost, though. He had brought the diagram with him on an unsuccessful chort hunt. With luck, it should be in the cave.
Finding The Chort Cave
The cave is southeast of Kaer Morhen, along a river. You will need to make your way there, without a road to speed you along. It's a fairly long journey that has no shortage of drowners and wolves to harass you.
Once you reach the cave, you will have to put the chort down. It still lives deep within the cave, waiting for the next challenger.
Chorts fight similarly to fiends without the hypnotic eye. Grease your sword with Relict Oil and be prepared to dodge your way around its slow attacks.
With the monster slain, use your Witcher Senses to find Berengar's supplies and Tor'haerne's crafting diagram. The quest will end here.
Tor'haerne is a particularly powerful silver sword. While it only has two slots for runes, it has armor piercing, increased critical hit chance and damage, as well as a chance to make its victims bleed.
