The Fallout games have long been obsessed with "uranium fever", but it feels like something even more contagious has hit the series recently - Ghoul fever. Ghouls have always been a key part of Fallout, but it feels like they're more present than ever before thanks to Amazon's adaptation giving us the glory of Walter Goggins as a Ghoul.

It's a little surprising that we've never seen it in the series before now, but Fallout 76 might just be the beginning for the mechanic.
Fallout 5 Could Get Playable Ghouls
While at Gamescom 2024, TheGamer's Deputy News Lead Rhiannon Bevan talked to Fallout 76's creative director, Jon Rush, about the game's upcoming playable Ghoul update. Beyond getting a ton of new information about how the feature is going to work (and how overpowered it is currently), Rush also discussed the possibility of playable Ghouls in future games - namely Fallout 5.
When asked if playable Ghouls could be included in Fallout 5, Rush was surprisingly honest and said that it's a possibility as long as fans respond to them positively and support their inclusion. If that does end up being the case, then Rush says that playable Ghouls in Fallout 5 would "absolutely be taken into consideration".
Possibly. I mean, if it's something that players love to do, then, yeah, absolutely. We make these games for the players so they have fun. [...] If the ghoul really supports that, then that would absolutely be taken into consideration. - Jon Rush
Of course, Fallout 5 is so far away at this point that pretty much anything can be true of it right now, but it does give Bethesda a lot of time to see how players react to being able to play as Ghouls and if it sticks around as a big fan-requested feature. Considering how prevalent Ghouls have been in 2024 alone, it seems likely that it's going to be a big hit, which makes it a shoo-in for Fallout 5.
